Cycle - The reporting cycle for which the data is submitted:
LEA, School -Local Education Agency, the unique seven-digit number assigned by the ADE to identify individual county, district, schools, cooperatives, and vocational institutions. The first two digits represent the county, the second two digits represent the district, and the last three digits represent the individual school, for example:
Vendor Number – The vendor identification number
Job Code – Number assigned by the ADE to each job assignment performed by a staff person. Only nurse job codes allowed at this time:

Degree Description – Description of degree
Number of Contracted Non-Employees – The number of Contracted Non-Employee Nurses (head count) per vendor, job code, degree and status
Contracted Non-employee Status - The employment status of the Contracted Non-Employee Nurses
F - Full Time P - Part time
FTE – The Full Time Equivalents (FTE). The FTE must be greater than 0.00
